Xanthogranulomatous inflammation is an uncommon affection of the female genital tract. It is characterized by accumulation of lipid-laden foamy macrophages intermixed with lymphocytes, giant cells, and plasma cells. A 30-year-old female with complaints of pelvic pain, admittet to the Obtetrics and Gynecology outpatient clinic. Histopathological examination of the ovarian material sent to pathology revealed areas of necrosis, lipid-laden macrophages, dense lymphocytes and neutrophils. It has been diagnosed as xanthogranulomatous oophoritis by the pathologic examination.
