Abstract

Amaç: Trigliserit-glukoz (TyG) indeksi ve trigliserit/HDL-kolesterol hücrelerinin (TG/HDL-c) üreme diyabeti (GDM) için tahminde değerlendirilmesi.

Yöntemler: 2023-2025 yılları arasında Tepecik Eğitim ve Araştırma Hastanesi'nde retrospektif bir vaka-kontrol çalışması yapılmıştır. 18-45 yaş arası tekil üremeler dahil edilmiş; üreme öncesi diyabet veya majör sistemik/metabolik hastalıklar dışlanmıştır. Gestasyonel Diyabet (GDM), Uluslararası Diyabet ve Gebelik Çalışma Grupları Birliği (IADPSG) kriterlerine göre 24-28 gebelik haftalarında 75 g oral glukoz tolerans testi kullanılarak teşhis edilmiştir. Açlık kan örneklerinden TyG indeksi ln(TG(mg/dL)×açlık glukozu(mg/dL)/2)ln(TG (mg/dL) × açlık glukozu (mg/dL)/2)ln(TG(mg/dL)×açlık glukozu(mg/dL)/2) ve TG/HDL-c hesaplanmıştır. Alıcı işletim özellikleri (ROC) analizi, %95 güven aralıklarıyla (CI) eğri altında alanları (AUC) tahmin etmek ve Youden indeksi kullanılarak optimal kesme noktalarını belirlemek için. Çok değişkenli lojistik regresyon analizleri, el anındaki yaş, doğum sayıları ve üreme haftası için ayarlamanın gerçekleştirilmesi.

Sonuçlar: Toplam 199 kadın analizi yapıldı (100'ü gebelik diyabetli ve 99'u kontrol grubu). Hem TyG (4,02±0,21'e karşı 3,84±0,18) hem de TG/HDL-c (3,92±2,12'ye karşı 2,80±1,29) gebelik diyabetli grubu anlamlı derecede daha yüksekti (her ikisi de p<0,001). TyG en yüksek seçme düzenleme yeteneği gösterdi (AUC 0,73; %95 CI 0,66–0,80), optimal kesme noktası 3,937 idi (duyarlılık/özgüllük: 0,67/0,67). TG/HDL-c ve trigliseritler orta düzeyde performans gösterdi (sırasıyla AUC 0,71 ve 0,70). Tedavi yapıldıktan sonra, hem TyG (0,1 birimlik artış başına OR 2,05; %95 CI 1,45–2,90) hem de TG/HDL-c (1 birimlik artış OR 1,52; %95 CI 1,24–1,87) GDM ile bağımsız olarak elde edilenler.

Sonuç: Rutin açlık laboratuvar testlerinden elde edilen TyG indeksi ve TG/HDL-c oranı, üreme diyabeti için orta düzeyde ayırt edicilik sağlamakta ve OGTT'ye ek olarak riskin çalıştırılmasını destekleyebilir. İleriye dönük dış düzen gereklidir.
